TeamHEALTH Staff Successfully Complete Outcome Star Facilitator Training

TeamHEALTH now have four facilitators across TeamHEALTH who are trained to conduct the Outcome Star Training for our staff.
To provide a basis that supports individual recovery planning, TeamHEALTH uses the Outcomes Star (the Star) as one of its tools to develop an Individual Recovery Plan.
The Star is an evidence-based tool that supports to prompt discussion to identify strengths of a participant, identifies what areas would make most positive impact on a participants life and to develop SMART goals that break down how they can achieve their goals.
Discussions occur around the 10 areas of the Star, these include: managing mental health, physical health and self-care, living skills, social networks and relationships.
TeamHEALTH operates within a psychosocial rehabilitation framework, with a focus on supports that are recovery focused, strengths based and that are culturally appropriate.
We encourage staff to lead the way by expanding their skill set and deliver this through their passion to support others and becoming a facilitator in the Outcome Star is a proficient way to showcase this.
